Semaglutide/Tirzepitide Consent

DISCLAIMER: We are prescribing the generic compound semaglutide or tirzepitide (SG/TZ), not the brand names Wegovy/Ozempic or Mounjaro/Zepbound. SG/TZ is an injectable prescription medicine used for adults with obesity (B.M.I. > 30) or overweight (B.M.I. > 27), who also have weight-related medical problems, to help them lose weight and keep the weight off.

Usage Instructions:
  • SG/TZ should be used with a low carbohydrate meal plan and increased activity.
  • SG/TZ should not be used with other SG/TZ containing products or GLP-1 receptor agonist medications.

  • Most common side effects include nausea, vomiting, diarrhea, constipation, stomach pain, headache, fatigue, upset stomach, dizziness, bloating, belching, gas, stomach flu, and heartburn.
  • Before using SG/TZ notify USA Telemedicine if you have any other medical conditions including: problems with your kidneys, depression, thoughts of suicide, mental issues, are pregnant or planning to become pregnant, are breastfeeding or plan on breastfeeding, or a personal or family history of thyroid cancer/tumor or MEN.
